Objectives not clearly defined

  • Lack of communication

  • Lack of clear direction

  • Lack of knowledge of the project environment (internal and external)

  • Insufficient product definition at the onset which can affect the quality of the product delivered

  • Poor statement of Work and No detailed work plan

  • Poor estimation of Time frame to complete the project and Poor cost estimate, causing cost overrun (remember the O2 project in Stratford!)

  • Lack of project progress reviews

  • Lack of project planning and lack or inappropriate assignment of resources

  • No project management methodology

  • Over-promising and under-delivering

  • Scope creep due uncontrolled requirements which are added during the project

  • Sponsors not implicated or lack of enthusiasm

  • Project risks not considered in the product definition

  • No continuous assessment of the viability of the project

  • Complexity of projects
